I know the film pretty much became a big target after the nailgun scene was featured predominantly on an early 1980s episode of "60 Minutes" that was condemnin exploitation films, which gave it it's "bad" reputation.

As for being banned, the only place I can think of was the UK, when it was caught up in the notorious "Video Nasties" scandel from around the same time.
