I say Batman is ultimately responsable for it because how can she forsee Matches Malone not showing up if Batman never bothered telling her the he is Matches.
He shouldn't have to tell her he's Matches - she hadn't earned that right yet - especially since she was not given access to those files. She had no right to hack into his files and as he never gave her permission to see those plans in the first place, he didn't have to explain to her any of the missing pieces (ie. Matches) to any of those plans. She was a noob and had not yet gotten to know everything there was to be about Robin, let alone Batman. What I meant by "forsee" is that seeing as she had just gotten the role of Robin, she should have known that there were many things about Batman and his techniques, procedures, etc., that she wouldn't been privy to - certainly not so early in the game.

Talia. That was the Tower of Babel storyline in JLA. Excellent tale - and again, he didn't just leave them around, Talia, at the behest of her father (again), hacked into his files for that information.

He let her be Robin on a trial basis - a trial that she clearly failed. There was no way that she would have been given access to, or been told everything about the Batman - and common sense should have told her as much.

Personally, I liked Spoiler and think that she should have been Batgirl when they first reintroduced that character.

No, like I said above, she hacked into the program, and there was no permission (express or implied) given to her by Bats to look at them.
